Model Release

All Fotility Photography & Videography (collectively, “Provider”) clients and models (collectively, “Models”) hereby agree to the following:

Consent

ALL booking clients (hereinafter referred to as the “Models”) must be consenting and over the age of eighteen (18) OR, if Model is under the age of eighteen (18), Model must a) be accompanied by a legal guardian at all times during their shoot, or b) produce a legal, notarized document from their legal guardian allowing them to have their photos and/or videos taken.

Model hereby assigns full copyright of any and all photos and/or videos taken by Fotility Photography & Videography, it’s representatives, licensees and assigns (hereinafter referred to as “Fotility”) during their session(s) to Fotility together with the right of reproduction either wholly or in part.

Right to Use & Publish

Model grants to Fotility the permission to any and all photos and/or videos taken by Fotility during their session(s), either separately or together, either wholly or in part, the perpetual and irrevocable and unrestricted right to use and publish videos and/or photos of Model where Model may be included for editorial trade, product advertising and such other fashion/business purpose in any manner and medium.

Unrestricted Use

Provider and licensees or assignees may have unrestricted use of these for whatever purpose, including advertising, with any retouching or alteration without restriction.

Indemnification

Provided Model names are not mentioned in connection with any other statement or wording which may be attributed to Model personally, Model undertakes not to prosecute or to institute proceedings, claims or demands against Fotility in respect to any usage of any and all photos and/or videos taken during their session(s). Model hereby releases Fotility from all claims and liability relating to images, videos or photos taken of them.

Security Cameras

For the safety of all clients and Fotility team members, Fotility maintains functioning security cameras inside the photo/video shoot areas of its studios. Model acknowledges that they have been made aware of all such security cameras in the Fotility studios.
